Acknowledgment of Membership Hold/Cancellation Terms
By signing this agreement, I acknowledge and agree to the following terms regarding my membership with VERDE Organic Body. I understand that in order to cancel or place my auto-pay membership on hold, I must provide a 30-day written notice by completing and submitting this form. I am responsible for any and all auto-pay payments that occur within this 30-day notice period, unless an authorized manager of VERDE Organic Body states otherwise in writing.
Membership Cancellations: Following the final payment date, I understand that I will no longer receive any membership benefits, including access to exclusive services, member rates, or priority booking.
I acknowledge that any remaining Sauna, Personal Training, and Massage sessions must be used within 60 days of the final auto-pay payment date, after which they will be forfeited. Any remaining unlimited-use services, including Yoga, Open Gym, and Group Training Sessions, must be used within 30 days from the final payment date. I understand that if I choose to restart my membership in the future, subsequent membership transactions may be subject to activation fees.
For personal training memberships, I understand that they are structured as three (3) month contracts that automatically renew every three months. If I cancel my membership before completing the current 3-month term, I may be responsible for an Early Termination Fee, which is calculated as 20% of the remaining balance of the contract. I authorize VERDE Organic Body to charge my card on file for the cost of any applicable early termination fees associated with this cancellation.
Membership Holds: I understand that VERDE Organic Body only offers hold options for Personal training and Massage memberships, and I agree to the terms associated with each.
Personal Training Hold: If I choose to place a Personal Training Membership on hold, I acknowledge that doing so reserves my regular training time slots and that I am required to pay 50% of my regular membership rate during the hold period. I retain any sessions purchased during the hold and may use them upon my return. My membership will remain at the 50% hold rate for the same number of months I was absent, until I have completed the sessions accrued during the hold. I understand that my regular personal training membership payments will automatically resume as indicated on the payment resume date provided.
Massage Hold: For Massage memberships, I understand that I am allowed to place my membership on hold for up to two (2) months per calendar year. During this hold period, I will not purchase any new Massage sessions. However, I may use any unused sessions on my account that remain valid. I acknowledge that my membership auto-pay will automatically resume on the specified resume date and that I am allowed only one 2-month massage hold per calendar year.
I understand that all prepaid sessions purchased under any active or canceled membership are final and non-refundable. If I choose to restart my membership at a later date, I may be subject to reactivation fees depending on the policies in place at that time.
By signing this agreement, I confirm that I have read, understood, and accepted all membership terms, including the 30-day cancellation notice, expiration timelines for unused sessions, the distinction between Personal Training and Massage holds, applicable early termination fees, and potential reactivation fees. I agree to comply fully with all policies of VERDE Organic Body.
