Auto-Pay Membership Contract Agreement – VERDE Organic Body
By signing this agreement, I acknowledge and agree to the following terms regarding my Auto-Pay Membership with VERDE Organic Body:
I understand that I will be charged a monthly fee via the debit or credit card on file.
These charges will occur automatically on the 1st or 15th of each month, depending on my sign-up date, and will continue indefinitely until I provide valid notice to cancel, as outlined below.
The undersigned acknowledges and agrees that a one-time, non-refundable activation fee of thirty-nine dollars ($39) will be charged at the time of membership activation. The exact charge date may vary depending on processing times and administrative procedures.
I understand that to cancel my membership, I must provide at least 30 days’ written notice by completing the designated cancellation form. Auto-payments will continue until this cancellation process is completed. I acknowledge that my membership will continue to renew each month until the proper cancellation procedure is followed.
This section applies only to memberships classified as Personal Training Reserved Slot Memberships:
I understand that personal training memberships are three (3) month contracts that auto-renew every three (3) months for an additional three (3) months. If I cancel my membership before the three-month contract is complete, I am responsible for an early termination fee, calculated as the difference drop-in rate versus member rate, of completed sessions in the contract term plus twenty percent (20%) of the remaining payments for the contract term. VERDE Organic Body has permission to charge my card on file for this fee once the cancellation form has been completed and signed in-studio.
This section applies only to memberships classified as the Ultimate Wellness Membership:
The undersigned acknowledges and agrees that upon enrollment in the Ultimate Wellness Membership, the initial membership period begins on the date this Agreement is executed. The fee for the initial month may be prorated based on the date of enrollment and the number of days remaining in that calendar month. Thereafter, membership fees will be billed on a recurring monthly basis in accordance with the terms of this Agreement, unless and until the membership is terminated pursuant to the applicable cancellation policy.
The undersigned further understands and agrees that the Ultimate Wellness Membership includes an initial three (3) month minimum commitment, excluding any initial prorated month. Upon completion of this initial term, the membership will automatically transition to a month-to-month basis unless otherwise cancelled in accordance with the cancellation policy.
This section applies to all auto-pay memberships:
All prepaid sessions purchased under this and all memberships are final and non-refundable.
If I choose to cancel my membership, I understand that I have 60 days from the final payment date to use any remaining sessions. Any unused sessions after 60 days will be forfeited.
VERDE Organic Body reserves the right to terminate my membership and auto-pay at any time due to inappropriate conduct, in which case any remaining prepaid sessions will be forfeited.
Receipts for my auto-payments will be provided upon request but are not automatically sent each month.
By signing this agreement, I confirm that I have read and fully understand all terms, including the activation fee, payment policies, cancellation procedures, and forfeiture of prepaid sessions. I agree to abide by all VERDE Organic Body policies and acknowledge that this membership will auto-renew until canceled according to the terms above.
