I, the parent/guardian, hereby give consent for my minor child to receive massage therapy services, personal training services, participate in fitness and/or yoga classes, or participate in any other services at VERDE Organic Body. I understand that the minimum age for participation for massage is 12, for yoga classes is 12, and for all fitness services is 16. I understand that VERDE Organic Body does not allow any participation of minors for Sauna services. I understand that if my child is under the age of 16, a parent or guardian may be required to be present in the Studio for the duration of the service. I understand that if my child is under the age of 13, a parent or guardian must be present in the massage treatment room for the duration of the massage service.
By signing below, I agree that I have read all the information above and have decided to provide consent to the above listed minor for services at VERDE Organic Body.
