VERDE STUDIO POLICIES + TERMS
GENERAL STUDIO POLICIES
All clients must register a valid Verde MindBody client account, including a valid debit or credit card on file to reserve and book appointments and services. All clients must complete health history intake form, liability waiver, studio policy agreement, COVID intake form, and any subsequent required documentation for participate in, or receive any of our services. Gift Cards DO NOT replace a valid credit/debit card on file required to make appointment reservations, no exceptions. All clients are responsible for disclosing health and medical conditions to studio staff (trainers, therapists, instructors) that includes but not limited to injuries, allergies, medications, or other medical information that may be pertinent to when receiving services or participating in training or classes at our studio. If clients choose to withhold pertinent medical information from studio staff, Verde and Verde staff will not be held liable for any reactions or injuries that are sustained.
MASSAGE RESCHEDULES + CANCELLATIONS
All Massage appointments sessions must be cancelled or rescheduled with a minimum of 24 hours notice. Failure to provide ample notice will result in forfeiture of prepaid sessions or a cancellation fee, as outlined below, charged to the card on file. No-Showing to any scheduled massage appointment is considered a late cancellation and will result in a cancellation fee or forfeiture of a prepaid session. VERDE Organic Body will allow same day rescheduling of appointments only in the event of: Emergency Situations, Personal Illness/COVID, Inclement Weather, and Therapists Absences. Any same day cancellation outside of these reasons will result in a cancellation fee being charged to client credit/debit card on-file.

PERSONAL TRAINING RESCHEDULES + CANCELLATIONS
All Personal Training sessions must be cancelled and rescheduled with a minimum of 24 hours notice. Failure to provide ample notice will result in forfeiture of prepaid sessions or a cancellation fee, as outlined below, charged to the card on file. No-Showing to any scheduled personal training appointment is considered a late cancellation and will result in a cancellation fee or forfeiture of a prepaid session. VERDE Organic Body will allow complimentary same day rescheduling of appointments only in the event of: Emergency Situations, COVID, Inclement Weather, Studio Closure, and Trainer Absence. Any same day cancellation outside of these reasons will result in a reschedule/cancellation fee being charged to client credit/debit card on-file or forfeiture of a prepaid session. All Personal Training sessions are subject to the following rescheduling guidelines and limitations within our Unlimited 10 Day/Same Trainer Reschedule Policy and Limited Complimentary Reschedule Policy.
Personal Training Reschedule/Cancellation Fees:

Unlimited 10 Day/Same Trainer Reschedule Policy: Personal Training Monthly Memberships, Single/Package sessions are allowed unlimited reschedules with the same trainer within 10 business days of the session date to be rescheduled. VERDE Organic Body does not guarantee future availability with the same trainer within 10 business days for these reschedules. In the event that a personal training session is not able to be rescheduled with the same trainer within 10 business days of original session date, the session holder has the options to 1) utilize a limited complimentary reschedule, retaining prepaid session to be used with any trainer at any time without expiration, as long as client remains as an active Personal Training membership auto-pay (detailed below) 2) pay the $30 Reschedule Fee, retaining the prepaid session to be used with any trainer at any time without expiration, as long as client remains as an active Personal Training membership auto-pay, or 3) to cancel the scheduled session resulting in forfeiture of a prepaid session on file.
Limited Complimentary Reschedule for Memberships Policy: Any Personal Training reschedules outside of 10 Day/Same Trainer Reschedule Policy for Personal Training Monthly Memberships are allowed limited reschedules to 25% of their monthly contract sessions. Complimentary Reschedules are defined as: rescheduling the session by retaining the prepaid session to be used with any trainer at any time without expiration, as long as client remains as an active Personal Training membership auto-pay, including roll-over sessions and sessions to be used within 60 days of proper termination of membership.
1x/Week Membership = 1 Monthly Complimentary Reschedule
2x/Week Membership = 2 Monthly Complimentary Reschedules
3x/Week Membership = 3 Monthly Complimentary Reschedules
Limited Complimentary Reschedule for Single Sessions/Packages Policy: Any Personal Training reschedules outside of 10 Day/Same Trainer Reschedule Policy for Single/Package Sessions are allowed limited reschedules to 25% of total scheduled pre-purchase single or package sessions scheduled within 30 days or calendar month. Complimentary Reschedules are defined as: rescheduling the session by retaining the prepaid session to be used with any trainer at any time, with a limited expiration of 30 days from session purchase date.
VIP OPEN GYM RESERVATIONS + CANCELLATIONS
All VIP Open Gym sessions must be scheduled independently by each client via the VERDE Organic Body website or mobile app. VERDE Organic Body will allow less than 24 hour notice cancellations for reschedule, by issuing the purchased session credit to client account, only in the event of: Family Emergency, Personal Illness, and Inclement Weather. Any cancellation made with less than 24 hours notice, and outside of listed reasons will result in forfeit of session purchase without refund. Failure to provide 24 hour notice will result in forfeiture of prepaid sessions, without refund. To qualify session credit on client account for approved cancellations, sessions must be cancelled or rescheduled with a minimum of 24 hours notice, by written notification via email to the studio info@verdeorganicbody.com. Sessions can be rescheduled by the client for use within a 30 day expiration period from date of purchase. Front desk is not responsible for rescheduling cancelled sessions. All initial and rescheduled sessions must be booked by the client independently by each client via website or mobile app. No-Showing to any scheduled VIP Open Gym reservation is considered a late cancellation and will result in forfeiture of a prepaid session. Sessions are only valid to be used within 30 days of date of purchase, and will expire within 30 days from date of purchase.
YOGA CLASS RESERVATIONS + CANCELLATIONS
VERDE yoga is vinyasa style yoga with room temperature (71-74 Degrees). All Yoga Class sessions must be scheduled independently by each client via the VERDE Organic Body website or mobile app. Clients are required to arrive to Yoga Class Sessions on time, and will not be permitted access to class once class session has started. Class Session start time is defined as the listed time format, for example 9:00am. If a client arrives late to a Yoga Class Session, it will be considered as a late arrival, without guarantee of refund and client will forfeit the pre-purchased session. VERDE Organic Body will allow less than 24 hour notice cancellations for reschedule, by issuing the purchased session credit to client account, only in the event of: Emergency Situations, COVID, Instructor Absence and Inclement Weather. Any cancellation made with less than 24 hours notice, and outside of listed reasons will result in forfeit of session purchase without refund. Failure to provide 24 hour notice will result in forfeiture of prepaid sessions, without refund. MASSAGE APPOINTMENTS RESERVATIONS + MEMBERSHIPS
Massage appointments must be scheduled in advance. Massage appointments can be scheduled same day, and can be scheduled in person, by phone, by text, through our website, or via the VERDE smartphone mobile app. Massage Memberships are charged on the 1st or 15th of each month for the cost of one or two prepaid sessions, per clients membership agreement form. Any unused prepaid sessions on the account will continue to roll over and will not expire as long as the client is an active Massage member. Client will receive member rate on all additional massage sessions outside of the prepaid sessions each month, for the duration of the active membership. VERDE offers non-discounted rates for individual sessions as well as packages for a discounted rate for all non-member clients.
PERSONAL TRAINING APPOINTMENTS RESERVATIONS + MEMBERSHIPS
Personal training appointments must be scheduled in advance, and paid in full prior to first training session date. All personal training sessions are semi-private with up to 2 clients per 1 trainer. Personal training memberships are charged on the 1st or 15th of each month for 4 weeks worth of sessions at $59 per session. Personal training memberships are designed on a 4 week schedule, and additional sessions may be required for purchase at member rate of $59 for long months that extend past 4 weeks. Personal Training membership contracts are 3 month contracts that auto-renew every 3 months. VERDE Organic Body will hold your requested schedule for the three months that you agree to pay for. Prepaid membership sessions do not expire as long as client remains active Personal Training membership auto-pay. To cancel a Person Training membership, client must provide a minimum of 30 days in studio notice with the studio provided form. Any membership cancellations made prior to the 3 month contracted end date of the contract are subject to an early termination fee. VERDE offers individual sessions and 4 packs at $69 per session, for all non-member clients. Individual personal training sessions and 4 pack personal training sessions will be scheduled out once purchase has been made for the number of sessions that are pre-purchased, and sessions expire 30 days after the date of purchase.
COMPLIMENTARY PERSONAL TRAINING 30 MIN CONSULTATIONS
At VERDE, we offer complimentary 30min personal training consultations with one of our Certified Personal Training team members, to help provide guidance for your fitness and health journey. Our complimentary consultations do require that we hold a valid debit or credit card on file to reserve the appointment. If you need to reschedule, we require a 24 hour notification for complimentary reschedules. In the event that you do not show up to your complimentary consultation, your valid credit/debit card on file will be automatically charged with a $25 no-show/late cancel fee. Any late cancellation or no-show to the complimentary consult may be subject to the $25 fee.

Failure to complete required forms prior to the scheduled service may result in refusal of service, without prior notification, and client may be held financially responsible for the full cost of the session time reserved.
MEMBERSHIP/AUTO-PAY CANCELLATION + HOLD POLICIES
All payments dates are either the 1st or the 15th of each month, and we will charge the card on file automatically on these dates for the cost of your membership or auto-pay as detailed in written contract completed and signed by each client. We require in 30 days in Studio notice to cancel any month to month memberships or auto-pays. Auto-pay membership sales are final and non-refundable. Clients are responsible to be aware and adhere to our 30 day in-studio notice required for membership cancellation and/or holds, and an physical cancellation form document must be completed in-person/in-writing to be considered notice of cancellation or hold.
Massage Membership Clients may put their Massage Membership or auto-pay on hold for free for up to 2 months per year with 30 day in-studio/in-writing notice. Massage Membership payments put on hold will automatically resume after the 2 month period ends, unless client contacts the studio and cancels the membership with an additional 30 day in-studio/in-writing notice to waive any further charges.
Personal Training Membership clients are not eligible to place auto-pay memberships on hold for their reoccurring membership payments. Early termination of any personal training membership autopay contract agreement will result in an early termination fee. The Personal Training Membership early termination fee is equivalent to; the total sum of $10 times the total amount of purchased sessions to date within the current contract, in addition to the added fee equivalent to 20% percent of the remaining contract amount owed. Personal Training Auto-pay Memberships renew automatically every three months, and will not be terminated unless the client completes the required in-studio/in-writing notice documentation.
Membership Cancellation Session Expiration Policy: When a client cancels a Massage or Personal Training auto-pay membership, any un-used sessions on the account will remain active on the account for up to 60 days following the membership final payment date, and un-used sessions must be used within 60 days of membership cancellation date. Membership priority booking status deactivates on the date of membership cancellation. Any sessions remaining on the client account after 60 days of membership cancellation will be forfeit.
GENERAL MINOR PARTICIPATION
All clients under the age of 18 will need a minor consent form signed by a legal parent and/or guardian prior to participating in or receiving services at Verde. All persons under the age of 16 must have a legal parent and/or guardian present while the service is taking place. Failure to have a legal parent and/or guardian present may result in refusal of service, without prior notification, and client may be held financially responsible for the full cost of the session time reserved.
MINOR MASSAGE PARTICIPATION
Minimum age requirement for massage is 12. All persons under the age of 16 may be required a parent present in the treatment room, or in reception area, during massage session.
MINOR PERSONAL TRAINING/VIP OPEN GYM PARTICIPATION
Minimum age requirement for personal training is 16. All persons under the age of 18 may be required to have a parent present in the fitness studio, or reception area, during Personal Training, and is required for or VIP Open Gym sessions.
MINOR YOGA PARTICIPATION
Minimum age requirement for yoga is 12. All persons under the age of 16 must have a parent present in the fitness studio, or reception area, during yoga session.
PRENATAL + ONCOLOGY CONSENT
All clients who are pregnant or who are diagnosed/have recently had active cancer must complete a prenatal or oncology intake form prior to participation in or reception of any VERDE services. Both the prenatal and oncology intake forms need to be completed and signed prior to participation in services. Failure to inform the studio and complete the needed forms prior to the scheduled service may result in refusal of service, without prior notification, and client may be held financially responsible for the full cost of the session time reserved.
EMAIL OPT-IN, CLIENT ACCOUNT INFORMATION + COMMUNICATION
All clients are required to complete and sign VERDE Organic Body’s health history intake form, liability waiver, studio policy agreement, COVID intake form, and any subsequent required documentation for participate in, or receive any of our services. Failure to complete required forms prior to the scheduled service may result in refusal of service, without prior notification, and client may be held financially responsible for the full cost of the session time reserved. We highly recommend but cannot require our clients to opt-in to automated account email communication within our client profile management system. VERDE Organic Body does not guarantee the accuracy or validity or accuracy of client submitted emails or phone numbers that are entered incorrectly or inaccurately into our client management system. In the event that a client does not enter a valid email or phone contact information into studio required client profile, VERDE Organic Body is not held responsible or liable for failure to deliver written or phone account or appointment reservation notification to client due to inaccurate or invalid information provided. It is the client's responsibility and agreement to obtain, maintain and be held responsible for collecting and all information regarding their client account, including but not limited to membership status, autopay contract status, session reservation status and transaction history for their personal records.
PHOTO POLICY
VERDE Organic Body reserves the right to take and use photos and videos from VERDE held at public studio events for any/all marketing campaigns used on sites or social media accounts run by VERDE, without limitation or obligation. If you wish to exclude yourself from photo participation, please communicate this to any event photographer at time of event, or simply remove yourself from any photo participation. This is included in our liability waiver, and must be agreed to for participation in services at VERDE.
LOST PROPERTY
All lost property at VERDE is kept at the studio for two weeks. After two weeks have passed, all lost items will be removed from the studio and donated. If you know that you have lost something at VERDE, you are required to contact the front desk in writing via email info@verdeorganicbody.com. Any confirmed/identified lost client items will be set aside/held for two weeks, requiring in-person client pickup.
GIFT CARDS
VERDE Organic Body sells two types of gift cards: massage service gift cards and non-massage gift cards. Gift cards must be purchased for the service you would like to gift, and cannot be transferred to other services, no exceptions. Gift Cards DO NOT replace a valid credit/debit card on file required to make appointment reservations, no exceptions.
ALL SALES ARE FINAL AND ARE NON-REFUNDABLE.
